We've had a Garmin III on the dashboard of the ambulance for several
years. Liked it since it could be velcro'd to the dashboard but could
fit into a pocket for search and rescue events. We've got pixels going
out on one of them and talking about replacing them with newer model.
Downloaded street maps are nice but not essential.

Most of what I've seen are good for auto use or for the back country,
but not both like the GPS III. Any thoughts on a product to look at?

If money is a problem, you might look at Magellan's
eXplorist series:

I got a GPS-V shortly before Garmin discontinued it. It came with City
Select v.7 which is adequate for my present needs. I bought Topo Canada
at the same time and it is about as good as it gets since it's the
latest 1:50,000 sheets where available and 1:250,000 sheets where
1:50,000 aren't available (mostly in the far north). If I were doing it
today, I would get a Garmin 76Cx.
