Whenever I start MapSource, it opens without any waypoints in the list.
I have a saved list of waypoints, how do I get the software to load
this file when it starts?

This is an easy one.

Create a shortcut to the *.gdb file on your desktop.

Double-click it.

Voilą...;=)

It is alleged that Christoph Bouthillier claimed:

I was thinking more in terms of a setting in MS, or a command line
option. The file had a generic "unknown type" icon.

If the file can be imported into MapSource and saved as a *.gdb file,
Christoph's suggestion a good way to do it.

The *.gdb files are MapSource's by default. But file type associations
are something you can control from Folder Options in the Control Panel.

Garmin either does not consider us smart enough to use command line
options or does not trust us enough to give us any info on them.

Yup, that's what I do too. I have a .gdb file that has all
of my waypoints, roadmaps, topos, routes, etc in it and just
double click it and away I go. It works well but I agree
that commandline options would be nice.
